Ortognatik cerrahi hastalarýnda farklý hareket miktarlarýnýn profil estetiði üzerindeki etkisinin deðerlendirilmesi

GÝRÝÞ ve AMAÇ: Ortognatik cerrahi uygulanan hastalarda farklý hareket miktarlarý sonucunda öngörülen profilin, estetik açýdan deðerlendirilmesidir.
YÖNTEM ve GEREÇLER: Farklý ortognatik cerrahi teknikler kullanýlarak tedavileri tamamlanan 7 hastaya ait preoperatif ve postoperatif latarel sefalometrik radyografilerden iki boyutlu siyah profil silüet görüntüleri hazýrlanmýþtýr. Ayrýca planlanan çene hareketlerinden sagittal yönde 2 mm daha az veya 2 mm daha fazla olacak þekilde preoperatif radyograflardan yeni profil silüet görüntüleri oluþturulmuþtur. Ortodontist, çene cerrahý, diþ hekimi ve hasta yakýnlarýndan oluþan farklý grup katýlýmcýlardan çenelerin farklý hareket miktarlarý neticesinde elde edilen profil silüet görüntülerini estetik açýdan skorlamalarý istenmiþtir. Her bir grupta 25 birey olacak þekilde hazýrlanan anket 100 katýlýmcý tarafýndan doldurulmuþtur.
BULGULAR: Tüm gruplarda en az estetik bulunan profil her bir hasta için ameliyat öncesi olmuþtur. En estetik bulunan profil tek çene operasyon uygulanan hastalarda post-operatif, çift çene operasyon uygulanan hastalarda ise daha az maksiller ilerletme ile birlikte daha fazla mandibular geriletme sonucunda öngörülen olmuþtur. Mandibula kaynaklý hafif prognati varlýðýnda uygulanan tek çene mandibular fazla geriletme, diþ hekimleri tarafýndan çene cerrahlarý ve hasta yakýnlarýna göre daha estetik bulunmuþtur. Mandibular retrognati gözlenen artmýþ alt yüz yüksekliðine sahip vakada daha fazla mandibular ilerletme ortodontistler tarafýndan anlamlý derecede daha yüksek oranda en az estetik þeklinde skorlanmýþtýr.
TARTIÞMA ve SONUÇ: Ortodontist, çene cerrahý ve hasta yakýnlarý estetik açýdan birbirleriyle büyük oranda uyumlu görüþ bildirmiþlerdir. Prognati gözlenen vakalarda daha fazla mandibuler geri itimin bir alternatif olabileceði düþünülebilir.

Evaluation of the effect of different amounts of movement on profile aesthetics in orthognathic surgery patients

INTRODUCTION: The aim of this study was to esthetically evaluate the predicted profile of patients undergoing orthognathic surgery as a result of different amounts of jaw movement.
METHODS: Two-dimensional black profile silhouettes were prepared from preoperative and postoperative latarel cephalometric radiographs of 7 patients whose treatments were completed using different surgical techniques. Additionally, new profile silhouettes were created from preoperative radiographs with 2 mm less or 2 mm more in the sagittal than the planned movements. The different participants including orthodontist, maxillofacial surgeon, dentist and laypersons were asked to score the profile silhouettes in terms of aesthetics. The prepared questionnaire was filled by 100 participants, with 25 individuals in each group.
RESULTS: The least aesthetic profile in all groups was preoperative for each patient. The most aesthetic profile was the postoperative in patients who underwent a single chin operation, while the predicted profile as a result of more mandibular set-back with less maxillary advancement in patients with double chin operation. Single-jaw mandibular over-setback in the presence of mild prognathia originating from the mandible has been found to be more aesthetic by dentists compared to maxillofacial surgeons and laypersons. More mandibular advancement was scored by orthodontists significantly higher as the least aesthetics in the patient with increased lower face height with mandibular retrognathia.
DISCUSSION AND CONCLUSION: Orthodontists, maxillofacial surgeons and laypersons reported a highly compatible opinion with each other in terms of aesthetics. It is thought that mandibular more setback may be an alternative in prognathia cases.
