Ýnfratemporal Locaya Deplase Olan Maksiller Gömülü Üçüncü Molar Diþin Ýntraoral Yaklaþým Ýle Cerrahi Çekimi: Bir Olgu Sunumu

Aðýz-diþ ve çene cerrahisi pratiðinde, gömülü maksiller üçüncü molar diþlerin çekimi en sýk uygulanan prosedürlerden biridir. En sýk karþýlaþýlan komplikasyonlar; tüberosite kýrýðý, diþ köklerinde oluþan kýrýklar, enfeksiyon, maksiller sinüs perforasyonu, bukkal yað dokusu prolapsusu ve alveolar osteittir. Bu komplikasyonlara oranla çok daha az sýklýkla, diþin infratemporal boþluk, maksiller sinüs, bukkal boþluk gibi anatomik boþluklara deplasmaný da gözlenmektedir. Bu olgu sunumu ise çocuk hastada infratemporal fossaya deplase olan maksiller gömülü üçüncü molar diþ germinin intraoral yaklaþýmla cerrahi olarak çýkarýlmasýný anlatmaktadýr. 12 yaþýnda sistemik olarak saðlýklý erkek hasta ortodonti bölümünden gömülü maksiller diþ germlerinin çekimi için cerrahi bölümüne yönlendirildi. Sað üst gömülü üçüncü molar germinin anatomik boþluða yer deðiþtirmesi sonucu çekim gerçekleþtirilemedi. Ýþlemin genel anestezi altýnda yapýlmasýna karar verildi. Infratemporal fossa ön sýnýrýnda ve zigomatik arkýn medialinde bulunan gömülü maksiller üçüncü molar germi, genel anestezi altýnda ilk operasyondan 3 hafta sonra cerrahi olarak çýkarýldý. Bu olgu sunumunda; potansiyel risk faktörlerini belirlemek ve bu komplikasyonun önlenmesi ve tedavisi hakkýnda bilgi toplamak hedeflenmiþtir.

Surgýcal Extraction Of Impacted Maxillary Third Molar Tooth Dýsplaced In Infratemporal Space With Intraoral Approach: Case Report

Extraction of impacted maxillary third molars is one of the most common procedures in oral and maxillofacial surgery practice. The most common complications are; tuberosity fracture, tooth and root fractures, infection, maxillary sinus perforation, buccal fat pad prolapse and alveolar osteitis. Compared to these complications, displacement of the tooth into anatomical spaces such as the infratemporal space, maxillary sinus, and buccal space is also observed much less frequently. This case report describes the surgical removal of the maxillary impacted third molar tooth germ displaced into the infratemporal fossa in a pediatric patient with an intraoral approach. A 12-year-old systemically healthy male patient was referred from the department of orthodontics for the extraction of impacted maxillary tooth germs. The extraction could not be performed as a result of the displacement of the right upper impacted third molar germ into the anatomical space. It was decided to perform the procedure under general anesthesia. The impacted third molar germ, which was found on the anterior border of the infratemporal fossa and medial to the zygomatic arch was surgically removed 3 weeks after the first operation under general anesthesia. In this case report; it is aimed to identify potential risk factors and to gather information about the prevention and treatment of this complication.
